I would REALLY prefer not to see this issue 'deferred', otherwise the site is going to turn into even more of a nightmare than it already is ;-)

I suspect the problem is that the content approval process doesn't make it clear that a piece of content is adding a new category?

can someone confirm if this is the case or not?

Everyone can add new categories. Altrought, reviewrs should not aprove what they think it is not a category.

The "Categories" box in the software package edit form contains what appears to be a lot of non-categories, such as:

"a boy and his catalog"

Where did they come from?
Why are they there? ;-)
